When it comes to skin treatments, it’s not just about what we use — it’s where and how deep we use it that matters. The skin is made up of several layers, and each plays a vital role in how we age, how we glow, and how we heal. Understanding these layers and targeting them appropriately can lead to long-lasting skin health — while avoiding unnecessary damage.
Let’s explore how we can treat each layer of the skin safely and effectively.
The Epidermis: The Surface Layer
What it does:The epidermis is our skin’s outermost layer — our first line of defence against environmental damage. It holds our skin barrier, which is essential for keeping moisture in and irritants out.
Best treatments:
Chemical peels (superficial)
Microneedling (superficial depths)
Medical-grade skincare (Vitamin C, retinoids, SPF)
Light-based therapies (LED)
Why we avoid aggressive damage here:Over-exfoliation, harsh acids, or strong treatments too frequently can compromise the skin barrier. This can lead to sensitivity, dryness, inflammation, and even premature ageing. Balance is key.
The Dermis: The Structural Support
What it does:This middle layer houses collagen, elastin, and blood vessels. It's responsible for our skin's bounce, firmness, and hydration.
Best treatments:
Collagen-stimulating injectables (e.g. Profhilo®, polynucleotides, Sculptra®)
Microneedling with skin boosters
Radiofrequency or ultrasound-based therapies
Mid-depth peels (in the right hands)
What to be cautious of:Overuse of deep chemical peels or laser resurfacing can permanently thin the dermis or damage melanocytes, especially in Fitzpatrick IV+. Stick to clinically proven treatments with minimal downtime and avoid overloading this layer with filler unless medically indicated.
The Subcutaneous Layer: The Foundation
What it does:This layer contains fat and connective tissue that provide volume and contour to the face.
Best treatments:
Biostimulatory injectables (e.g. Sculptra®, Radiesse® in hyperdilute form)
Fat grafting or strategic filler use (only when volume loss is clearly indicated)
Why caution is crucial:Permanent fillers or inappropriate deep filler placement can stretch or damage ligaments and skin integrity over time, especially in younger patients or when used preventatively. This can lead to a distorted appearance as natural ageing progresses. For many patients, stimulating the tissue to rebuild itself (rather than adding bulk) is the better long-term strategy.

It all comes down to your individual skin goals, age, and lifestyle. The best approach is a layered one — think of it as feeding your skin from the top down:
Hydrate and protect the surface.
